Hemo dialysis: <70 kg: 1000 mg iv oplaaddosis gevolgd door 500 mg iv onderhoudsdosis bij dialyse 70-100 kg: 1250 mg iv oplaaddosis gevolgd door 750 mg iv onderhoudsdosis bij dialyse >100 kg: 1500 mg iv oplaaddosis gevolgd door 1000 mg iv onderhoudsdosis bij dialyse

CAV / VVHD: Continu: normale oplaaddosis gevolgd door 1000 mg continue infusie per 24 uur; spiegel na 24 uur. Intermitterend: 1dd 15mg/kg iv; na eerste gift topspiegel en voor 2e gift dalspiegel
