Comments:

Gentamicine bij endocarditis is de dosering 3 mg/kg 1dd.

Obesitas:
1. Bij een gewicht tot 100 kg: gebruikelijke standaarddosis
2. Bij een gewicht tussen 100-221 kg: doseer 1e gift in mg/kg TBW op basis van CKD-EPI (in ml/min/1.73 m2). Verlaag de d osering met 25% voor IC patienten
CKD-EPI > 120: 1e gfit 100% van standaarddosis
CKD-EPI 90-120: 1e gift 80% van de standaarddosis
CKD-EPI 60-90: 1e gfit 60% van de standaarddosis
CKD-EPI 30-60: 1e gift 40% van de standaarddosis per 24-26u
CKD-EPI < 30: 1e gift 30% van de standaarddosis per 36-48u
Doseer na 1e gift ogv spiegels volgens lokaal TDM-protocol

Hemo dialysis: 1e gift standaarddosis voor dialyse. Doseer vervolgens ogv spiegels volgens lokaal TDM protocol

CAV / VVHD: Normale aanvangsdosis, vervolgens op geleide van spiegels volgens lokaal TDM protocol
